Medicare Payment for Health Care Home Services in Minnesota to
Begin in October
Medicare payment for health care home services in Minnesota will
begin in October. This new reimbursement for coordination of
care is an additional incentive for practices to seek certification
as health care homes.
Through Minnesota's participation in the Medicare Multi-payer
Advanced Primary Care Practice (MAPCP) demonstration, certified
health care homes will be reimbursed for care coordination services
provided to fee-for-service Medicare beneficiaries beginning
10/1/11.
"I'm excited that Medicare has acknowledged the potential of the
patient-centered medical home through this demonstration project,"
said MAFP Immediate Past President Terry Cahill.

Certified health care homes that wish to participate in the MAPCP
demonstration and have not already notified MDH should contact
