Patient Education

Leading a Healthier Life with Chronic Conditions
This six-week workshop is for patients with chronic and complex health conditions who want to learn how to manage their illness, enhance their day-to-day living and lead a healthier, more satisfying life.   Led by trained lay leaders, participants receive fifteen hours of classroom learning.  Stanford University’s School of Medicine developed this evidence-based workshop, which is now available to Minnesotans through a collaboration of local health organizations.

Resource Guide for Seniors
Designed for senior citizens, the guide lists patient assistance programs that offer free or low-cost prescription drugs and promotes the Senior LinkAge Line for seniors in need of in-home community services to maintain a more independent lifestyle.

Professional Education

How to Work Effectively with Trained Interpreters
This one-hour education session is for family physicians, family medicine residents and medical students who want to enhance their communication with patients who have limited English proficiency.  The program is a partnership of the MAFP Foundation, Minnesota Medical Association, Interpreters Stakeholders Group and MAFP Academic Affairs Committee.

Protect Your Patients Against Pertussis (Whooping Cough)
