MAFP Foundation: A robust agenda for family medicine.
Your gift will help fund patient and professional activities,
educational programs and research grants to Minnesota's next
generation of family physicians.
What the MAFP Foundation is doing: -
Providing grants to medical students
- Improving access to health care in underserved rural and urban
communities
- Reducing healthcare disparities
- Training residents to see how practice-based research can impact
patient care
- Working in partnership with other organizations for even greater
impact for the health of all Minnesotans
Resident Research Grant Program
Application deadline:
February 1, 2013
CLOSED
Apply for a $3,000 grant to gain real-world experience with
practice-based research, develop skills in basic research methods,
and improve your understanding of how of family medicine research
can impact patient care. Design your own study or work with other
residents to design a team research project at your residency
program. Recipients have up to 24 months to complete their study.
A final paper and presentation at a future MAFP Research Forum are
required.
Before applying, residents must consult with a family physician
research mentor. If you need help finding a mentor please contact
the
MAFP Foundation. Applicants and mentors must be members of the Minnesota Academy of Family Physicians.

implementing a research or non-research oriented project that is
important to family medicine. Students and members must be members of the MAFP
(free for students).

two areas of obesity and rural health or healthcare.

Pertussis (Whooping Cough) Vaccination
Campaign

Clinics participated in the statewide Pertussis
Protection Campaign to limit exposure and the potential to spread
pertussis to patients.
Rural Physician Loan Forgiveness Program
Physician loan forgiveness helps Minnesota’s communities and
physicians. With loan forgiveness assistance, a physician gets help
paying down educational debt and the community gets a doctor. The
physician makes a commitment to practice in the community for a
minimum of three to four years. Once there, most physicians choose
to stay much longer. The MAFP Foundation awards a grant to the
Minnesota Medical Association Foundation in support of this
program.

Chronic Disease Self-Management Program
Living Well with Chronic Conditions: Put Life Back in
Your Life!
Through a funding
partnership with the Minnesota Department of Health, the MAFP
Foundation offers mini-grants and technical assistance to family
medicine clinics implementing the Living Well with Chronic
Conditions workshop for patients with health conditions such as
diabetes, arthritis, high blood pressure, heart disease, chronic
pain and anxiety.

During the six
week series, participants find information and practical skills for
managing pain and fatigue; healthy eating; physical activity; making
informed treatment choices; and talking with physicians and family
about health concerns.
Most importantly,
participants will gain confidence, motivation and find the practical
solutions they need to manage the challenges of living with a
chronic health condition.

and clinics may contact the MAFP Foundation to learn more.

Medica
reimburses providers for
evidence-based health promotion programs such as the chronic disease
self-management program.
Enhancing
Communication with Patients with Limited English Proficiency
A
professional education program designed to reduce healthcare
disparities and improve healthcare outcomes for patients with
limited English Proficiency. To schedule this education program at
your clinic, medical school or family medicine residency program,
contact the MAFP Foundation.
