Foundation Programs

Conference on Practice Improvement December 4-7, 2008 in Savannah, Georgia

Family medicine residents are invited to apply for a $1,000 educational grant from the Minnesota Academy of Family Physicians Foundation to attend the 2008 conference.  The grant covers conference registration and limited travel reimbursement.

Don’t miss the only national meeting of its kind targeting family physicians and other primary care professionals concerned about educating patients for better health outcomes.

Conference Theme:  Building a Medical Home

You cannot build a home from nothing. You need a plan. You need resources. Sometimes you need the experienced craftsmen and women with special talents and perspectives to show you the potential of what your home can become. The 2008 Conference on Practice Improvement is designed to help any practice setting begin or continue the work to create a medical home in their practice.

The conference seeks to provide a forum for the reflective redesign of practice roles and processes through the team approach. Attendees are encouraged to bring interdisciplinary teams from their practices.

The conference is sponsored by the American Academy of Family Physicians and the Society of Teachers of Family Medicine.

Call for Presentations

Submit your presentation proposal now for the 2008 Call for Presentations. All submissions are accepted electronically through an easy online system. Deadline for submissions is March 10, 2008.
