MINNESOTA ACADEMY OF FAMILY PHYSICIANS
FOUNDATION 2008 GRANT GUIDELINES
Overview
The mission of the Minnesota Academy of Family
Physicians Foundation (MAFP Foundation), as the philanthropic arm of
the Minnesota Academy of Family Physicians, is to support family
physician health care delivery, research and education; and to
promote the specialty of Family Medicine. The MAFP Foundation
fulfills its mission by identifying programs, projects and community
partnerships that advance the work of the MAFP Foundation. The MAFP
Foundation supports innovative and high interest programs that
reflect our mission.
Selection Criteria
- Grants are open to medical and health related organizations
in Minnesota that are commissioned by or employed by organizations,
state and local health departments, and regional or state agencies.
- Grant proposals must be consistent with the MAFP Foundation’s
mission and goals.
- Grant proposals must fit within the scope of the specialty of
Family Medicine.
- Grant proposals must demonstrate collaboration within the
medical and general community.
- Applicants must agree to provide a written summary report
(and photos if possible) at the conclusion of the funded project
describing the outcomes, successes, and challenges.
Selection Process
Grant proposals will be reviewed by the MAFP Foundation’s
Executive Director and Board of Directors. Final approval is
required from the MAFP Foundation Board of Directors.
Application Instructions
Complete the MAFP
Foundation
Grant
Application. Include a cover letter (no
more than three pages excluding
budget
worksheet
and other attachments) that clearly states:
- Name and address of organization applying for a grant
- Contact person, title, telephone, email address, and website
- Description of your organization's mission including geographic area served and other background information
- The organization's tax ID number
- Board of Directors (if applicable)
- Description of project, including:
- How does the project align with the mission of the MAFP Foundation
- Funding amount being requested
- Objectives
- Target audience
- Specific activities to meet the project's goals
- Other medical and health groups that are collaborating on the project
- Anticipated barriers/challenges and strategies to overcome them
- Evaluation techniques used to measure impact
- Project timeline
- Itemized project budget (may use
budget worksheet)
- Qualifications of the organization/individuals to implement this program successfully
- Other grants or awards underway or submitted for this project
