Chronic Disease Incentive Program

The Minnesota Department of Human Services has been an active partner and proponent of incentives to improve health care outcomes, in particular for optimal chronic disease care to patients with diabetes and cardiovascular disease.   For Minnesota Health Care Program enrollees in managed care, the Department of Human Services participates in the Bridges to Excellence Program, and for Fee-For-Service enrollees, there is a comparable chronic disease incentive program.  Providers may earn $250 up to twice in 12 months for providing optimal care to eligible patients. Providers are encouraged to review the information and submit the necessary data to earn the incentives.  Here is a link to the Minnesota Health Care Program Provider Manual web site.  We ask that you please distribute the information to appropriate staff within your organizations who will be submitting requests for the reward payments.
