Family Physician of the Year Award
This award is presented annually to a family physician who represents
the highest ideals of the specialty of family medicine, including caring,
comprehensive medical service, community involvement and service as a role
model.
The MAFP seeks nominations from the public for
this award. Nominations may also come from colleagues and administrators.
Nominations must be in writing. Nominators should describe the candidate’s
outstanding personal and professional qualities, as well as cite examples of the
nominee’s involvement in his/her community.
Criteria
Nominees must be MAFP members and spend
at
least fifty-percent of his/her time in direct patient care.

physician's membership status.)
Caring: What makes this individual special? How
have they demonstrated compassion?
Comprehensive Medical Service: Describe
the quality of care provided by the individual. How do they care
for you as a whole person? Do they care for other members of your
family?
Community: Where does this individual
volunteer? How does this individual make a difference in the
community beyond their job in the clinic?
Service as a Role Model: Does this
individual serve as a mentor to students/residents? Is this
individual a role model?

Nomination deadline for 2012:
December 5, 2011
(Nominations accepted between August 1, 2011, and
December 5, 2011)
Once the nomination deadline has passed, additional letters of
support for each of the nominees will be accepted until December
15th. However, no new nominations will be accepted
beyond the initial deadline. *Update - Any
letters of support received before NOON on Monday, December 19,
2012, will be accepted.

All letters are carefully considered by members of a task force who will
select up to four finalists. The MAFP Board of Directors will choose one of the
finalists to receive the Family Physician of the Year Award. The award will be presented during the MAFP Annual Meeting/Spring
Refresher in April 2012.
