Those who do not wish to register online can mail a completed registraton form and payment to the MAFP office at 600 S. Highway 160, Suite 1680, St. Louis Park, MN  55426.  Forms can also be faxed to the MAFP office at 952-542-0135.  (Participants who register by fax will be invoiced.)  Printable Registration Form (PDF)

Cancellation Policy
Only cancellations received by April 12th will be refunded, minus a $35 service fee.
